Health History Form

This is our introduction to you and your history. Your detailed and thoughtful responses will help us to use our time in the clinic more effectively. Please bring this form with you to your first visit.

This form gives authorization to use or disclose protected health information.

Lab work

If you have previous lab work, please bring a copy to your appointment or arrange to have a copy sent to our office prior to your visit.

Please eat prior to your appointment

Make sure you eat your regular meals and drink enough water before come to our office. We do not suggest you to have acupuncture treatment with empty stomach and dehydration.

Cancellation Policy

If you find it necessary to cancel your appointment, please give us 24 hour notice.

Appointments that are cancelled without due notice will require $70 payment prior to scheduling a new appointment time. If you miss the second New Patient appointment without 24 hours notice, the $70 will be applied to a New Patient missed appointment fee.
